Healthcare Surrogate, Explained Simply

In plain English: a Healthcare Surrogate form names the person who speaks for you at the hospital — someone you trust to make medical decisions if you can't speak for yourself. In Florida it must be signed in front of two adult witnesses. I coordinate the signing so it's done right the first time.

Common questions

Does this form need to be notarized in Florida?

Honest answer: Florida law requires two witnesses, not a notary. But many hospitals, facilities, and families prefer it notarized for extra certainty — and since I'm there anyway, one visit covers both. I'll never charge you for something the law doesn't require without telling you first.

Who can be a witness?

Two adults. The person you name as your surrogate can't witness, and at least one witness must not be your spouse or a blood relative. Neighbors work great. If gathering witnesses is hard, mention it when you message and we'll sort it out.

Can my spouse or child be my surrogate?

Yes — that's who most people choose. They just can't also serve as a witness to your signature.

Can you come to the hospital before surgery?

Yes. Pre-surgery signings are a regular part of my work. Message me your timing and I'll plan around the hospital's schedule.

How do I pay?

Whatever is easiest for you when the job is done. Daytime appointments need no deposit. Evening and weekend appointments are confirmed with a $25 deposit that counts toward your total — I'll send you a secure payment link.
