General Affidavit, Explained Simply

In plain English: an affidavit is a written statement you swear is true — signed under oath in front of a notary. Banks, courts, the DMV, and insurance companies ask for them all the time. Unlike some documents, this one does require a notary: the oath is the whole point. That's me.

Common questions

Do I sign the affidavit before you arrive?

No — bring it unsigned. You take the oath and sign in front of me. An affidavit signed ahead of time can't be properly notarized as a sworn statement.

What ID do I need?

A valid, unexpired government photo ID — driver's license, passport, or state ID. If ID is a problem, Florida allows two credible witnesses instead; mention it when you message.

Can you write the affidavit for me?

No — I'm a notary, not an attorney, so I can't draft it or tell you what to say in it. I administer the oath and notarize your signature. If you need the document written, an attorney or the requesting agency's own form is the way to go.

Can you come to a hospital or nursing home?

Yes — that's a regular part of my work in the Sun City Center area, handled with patience and care.

How do I pay?

Whatever is easiest for you when the job is done. Daytime appointments need no deposit. Evening and weekend appointments are confirmed with a $25 deposit that counts toward your total — I'll send you a secure payment link.
